Supporting carers to identify options for care

Our Carer Coordinator provides information and support to unpaid carers. This includes people who provide care to family members or friends in an informal capacity. We give carers time to discuss their caring role and support them to find solutions to issues they may be having in relation to hospital discharge, accessing health and social care services or general information on coping with the people that they are caring for. We work in collaboration with health and social services staff, the voluntary sector, the Dudley Carers Alliance members and carers themselves. We work with carers 1:1, carers and their families can also come along to drop-ins in front reception and on the wards and also by attending meetings with hospital and community staff to raise awareness of carers issues. We also support carers with Carers Tea and Chat at the hospital, an informal drop-in for carers to meet each other and find support from local organisations.


Our Carer Co-ordinator supported Paula and her family to identify options for care


Our Carer Coordinator was contacted by Paula who had been supporting her father who was caring for her mother after being recently discharged from hospital. The family had declined support on discharge from hospital, however, Paula felt worried that they weren’t managing and that her father wasn’t coping very well with the situation. Our Carer Coordinator met with the family and completed a carers first contact assessment to identify needs.


The couple were referred to the urgent care team for a package of care to be implemented twice daily and also to provide a sitting service to give him some time out from the caring role.


Carers were provided twice daily to assist Paula’s mother with washing and dressing in the morning and at bedtime which relieved her father of some of the pressure.  When Paula’s mother was also diagnosed with dementia our Carer Coordinator linked the family to appropriate services.
